Intake Survey 2013 is launched

Background

In order for the University to provide you with a better UG education, the Student Affairs Office (SAO) and Center for Enhanced Learning and Teaching (CELT) jointly launch an annual Intake Survey to collect information about your background, special educational needs, pre-university education, goals and expectations of your university education. This is a compulsory event for all Healthy Lifestyle Course (HLTH1010) students. Upon successful completion of this survey, students will attain 1 attendance hour for the Intake Survey in the Wellness module. Students should have received the invitation email sent from HLTH1010 course office. Please follow the instructions below and complete the Intake Survey.

Instructions for completing the Intake Survey

  1. Download this Excel file and check which questionnaires you should complete.
  2. The Intake Survey has three parts and you need to complete one questionnaire in each part (i.e. 3 questionnaires in total).

  3. When you access the questionnaires, you will be asked to login using your ITSC username and password.
  4. If you encounter the following message, most probably you have clicked the incorrect questionnaires. Please check this Excel again.
  5. Upon your completion of all three questionnaires, you can gain ONE course hour for HLTH1010. The course hours will be uploaded to your course progress by 18 October 2013. Please check your participation record here.
